LIPOSUCTION

Liposuction is the second most frequently performed cosmetic surgical procedure in the world after eyelid surgery. This provides improvement of body contour, incuding a reduced abdominal circumference, a flat anterior abdomen, and better waist defintion (S-shaped abdomen). Liposuction has evolved in recent decades: the use of smaller-sized cannula, the use of new technology such as ultrasound- and laser-assisted liposuction and the use of fluid infiltration in fat tissue (tumescent techniques) have improved outcomes.

What is Liposuction?

In short, liposuction is the removal of fat deposits in the body by using suction machine. This process is much less complicated than fat transfer to face, breasts or buttocks so you have to discuss with the doctor before in case you want to have it done.

A traditional liposuction procedure involves four basic steps:

  1. IV (intravenous) drip sedation with vital signs and O2 monitoring is applied to put u to sleep, the operative hours depend on the number/areas of your procedure(s).
  2. A 0.5-1 cm incision or incisions is made near the selected area and a thin cannula is inserted through the cut at skin. Then 1-2 litres of mixed anesthetic with a small amount of adrenaline in normal saline solution is infused through the cannula to each area. This is called "Tumescent technique" to provide tissue numbness and avoid bleeding during suction.
  3. Fat is sucked out into the collecting bottle via the suction cannula.
  4. The incisions are closed with absorbable or non-absorbable suture and padding is applied. Compression garment need to be worn immediately to reduce postoperative swelling and bruising and continued for at least 2 months for smooth and tightening results.

Despite the fact many people diet and exercise religiously, sometimes it's physically impossible to get rid of fat deposits in certain areas of the body such as the abdomen, thighs, arms or buttocks.

Liposuction is No Shortcut to Weight Loss

Liposuction is used to improve body contouring by removing localized fat deposits that may not respond to weight loss, such as exercise and dieting. It's extremely important to stress that liposuction isn't a shortcut to weight loss, and it's recommended primarily for healthy adults within 10-20 pounds of their ideal weight looking to remove stubborn pockets of fat and shape and contour their body. The primary indications include fat removal at abdomen, flanks, thighs, calves, buttock, arms and neck. Common Types of Liposuction

Liposuction has become one of the fastest, most effective and safest cosmetic surgical procedures available today, and much of that is due to the technological advancement of both the techniques and the equipments

Traditional Liposuction

Tumescent (traditional) liposuction is the most common type of liposuction. The basic process is surgeon will insert the cannula through the incision and inject lidocaine (anesthetic) and epinephrine (Blood vessel contraction) into the fatty tissue, causing it to becomes swollen and firm, which is called "tumescent." This helps break up the fat deposits for easier suctioning, and minimal bleeding.

 
Vaser (Ultrasonic) Liposuction
 
Vaser liposuction is an increasingly popular alternative to traditional liposuction. Vaser lipo technology involves using ultrasonic sound waves to gently loosen the fat for easier removal. Tumescent liquid is still injected into the body, but instead of the cannula ultrasonic probes use vibration to break the fat cells apart. Once the fat is emulsified, a small cannula is used to suction out the fat cells. Some of the local anesthetic remains in the tissue and that helps to reduce post-procedural pain. Vaser liposuction is considered gentler on the body and the remaining cells in the treatment area than more conventional liposuction methods. 
 
Anesthesia for the Procedure
With General Anesthesia , a machine is breathing for you and you have a tube down your throat or nose.
With intravenous (IV) anesthesia you are breathing on your own and there is no tube in your throat or nose. U will sleep well during the procedure. Recovery from IV sedation is also much easier than General Anesthesia. Most people experience much less nausea with IV sedation. 

 

Tumescent + Intravenous Sedation = Less Worries than General Anesthesia With tumescent anesthesia and IV sedation, I do not have to worry about the the many factors that are present with general anesthesia: length of the surgery, post operative nausea and pain, the need for IV, and possibility of blood clots to leg veins are just not there.

You might think that this is not a lot of weight taken off and you would be right because liposuction is not a weight reduction technique. But, the changes in your figure can be dramatic, because liposuction is a great contouring technique.

Preoperative care

  1. Patients should quit smoking for 1-2 weeks before surgery
  2. Blood tests including blood sugar should be performed preoperatively
  3. Patients over 45 years of age, a cardiac evaluation (ECG) and chest X-ray should be done and the results should be shown to the doctor

Postoperative care

  1. Early ambulation is essential to lower the risk of venous thromboembolism
  2. Compression dressings can be removed after 1 week. Compression garment are worn at all times for the first 3 weeks, then nightly for an additional 2 months
  3. Most patients can return to work in 3 to 5 days and regular activities in 2-3 weeks after surgery and exercise 1 month after surgery
  4. Massaging at treated areas starting 2 weeks to encourage lymphatic drainage and prevent uneven and wavy tissue after liposuction. This should continue until the tissues become supple, which will occur approximately 2 months after surgery
  5. Taking analgesics, antibiotics and antiinflammatory medications for 7 days
  6. Patients should control their weight to guarantee best results

 

* The cost of Traditional liposuction at our clinic is 15,000 THB per area and the procedure takes about 1-1.5 hours per area using intravenous sedation and anesthetic injection
 
* The cost of Vaserlipo is 30,000 THB per area. Both arms, outer thighs, inner thighs, both calves, underchin and underjaw, back, buttocks and love handles are considered one area each. Upper and lower abdomen are separated into 2 areas. The procedure takes about 1.5-2 hours per area for Vaser liposelection.
